Somatotopic organization of motor fibers in the corona radiata in monoparetic patients with small subcortical infarct.

چکیده
BACKGROUND AND PURPOSE The somatotopic organization of the corticospinal fibers is of importance because it is related to certain stroke syndromes. Although it has been suggested that motor fibers are somatotopically arranged in the corona radiata, the evidence is still insufficient in human. METHODS The relative anteroposterior and mediolateral location of the lesions was measured on T2-weighted MRI in 28 patients who developed isolated motor deficit limited to the arm, leg, or bulbofacial muscles after a small corona radiata infarct. RESULTS The location of the lesions associated with bulbofacial, arm, and leg paresis showed anterolateral-to-posteromedial distribution. CONCLUSIONS The results suggest that motor fibers subserving the bulbofacial, arm, and leg muscles are somatotopically arranged at the level of the corona radiata.

Diffusion-tensor imaging-guided tracking of fibers of the pyramidal tract combined with intraoperative cortical stimulation mapping in patients with gliomas.
OBJECT The goal of this study was to use diffusion-tensor (DT) magnetic resonance (MR) imaging to track fibers combined with cortical stimulation mapping to delineate descending motor pathways. Subcortical localization of motor pathways in relation to a glioma may provide critical information to guide tumor resection and prevent surgical morbidity. METHODS Eleven adult patients harboring glio...
متن کاملEffect of lesion location on upper limb motor recovery after stroke.
BACKGROUND AND PURPOSE The purpose of this study was to assess the effects of stroke involvement of primary and secondary hemispheric motor systems and corticofugal tracts on arm and hand recovery. METHODS Forty-one patients participating in an inpatient stroke rehabilitation database, admitted 17+/-2 (SEM) days after initial unilateral hemispheric ischemic stroke, with neuroimaging studies p...
متن کاملIpsilateral hemiparesis caused by a corona radiata infarct after a previous stroke on the opposite side.
Ipsilateral hemiparesis after a supratentorial stroke is rare. However, the role of the reorganization of the unaffected hemisphere in recovery after a stroke is poorly understood. Two patients developed ipsilateral hemiparesis after a left corona radiata infarct. Both of these patients had previously experienced contralateral hemiparesis after a right-sided supratentorial stroke. Functional ma...
متن کاملExamining Subcortical Infarcts in the Era of Acute Multimodality CT Imaging
BACKGROUND Lacunar infarct has been characterized as small subcortical infarct. It is postulated to occur from "in situ microatheroma or lipohyalinosis" in small vessel or lacunar mechanism. Based on this idea, such infarcts by lacunar mechanism should not be associated with large area of perfusion deficits that extend beyond the subcortical region to the cortical region. By contrast, selected ...
متن کاملMotor outcome according to the integrity of the corticospinal tract determined by diffusion tensor tractography in the early stage of corona radiata infarct.
Diffusion tensor tractography (DTT) is useful for exploring the state of the corticospinal tract (CST). An accurate estimation of the integrity of the CST in the early stage of a cerebral infarct would enable a determination of motor recovery. DTT was performed to classify CST integrity following a corona radiata infarct to evaluate if the procedure could characterize the motor outcome of the a...
